RN Care Manager

- Advocate for older and/or disabled adults and their loved ones to help make decisions related to healthcare, aging and long-term care planning which will

- Conduct, develop, monitor, and manage care plan and assessments to identify

- Onboard and sign up new clients, educating them on how our Care Managed Home Care (CMHC) services will help them age in place
- Review financial, legal and/or medical issues and offer referrals and recommendations to specialists and other professionals
- Focusing on avoiding future problems and conserving assets
- Assist with determining the level of care needed and moving a client to a more appropriate facility or the arrangement of in-home care
- Act as liaison to responsible parties and professionals
- Facilitate our Care Managed Home Care model by:
- Manage intakes, complete on-site safety assessments and complete care plans for new clients
- Provide coaching for caregivers with new clients to ensure success
- Overseeing the orientation and training to new and existing caregivers
- Complete documentation requirements accurately and timely
- Participate in marketing efforts with the goal of educating the public, cultivating clients, and securing new clients
Compensación: $30.00 - $35.00 per hour
