highest caliber care for our patients. We are looking for a licensed MSW or RN Care Manager to
service both seniors and adults with physical disabilities, mental health issues, dementia, and
traumatic brain injuries.
Assessment of new patients and maintain ongoing patient evaluations.
Create individualized care plans based on specific patient needs.
Respond and handle patient crises in a professional and timely manner.
Develop coherent, legible, and grammatically correct assessments, care plans, and
monthly reports on a timely basis.
Document all patient interactions in our online software program.
Represent the organization through positive interactions in the community.
$45 per hour
$1000 hire-on bonus after first 90 days!
schedule how YOU see fit, but does requires availability during outside office hours including
outside normal business hours, holidays, and weekends, as business requires.

state of Oregon, required; LCSW not required
Valid drivers license, personal vehicle, and proof of vehicle insurance, required
Ability to speak, read, and write in English, required (bilingual applicants are strongly
welcomed to apply!)
At least 1+ years' experience working with seniors and/or adults with mental illness,
preferred
Flexible schedule to allow for working occasional weekends and evenings, preferred
Ability to work with challenging clients and respond to emergency situations
