- Compensation: The anticipated base compensation for this position is between $60.00–65.00 hourly. Offered compensation will be determined based on education and experience.
- Location: Client homes throughout the South Bay region of the Bay Area (Santa Clara County)
- Hours: 30+ hours per week

What You’ll Do:
- Conduct comprehensive, holistic in-home assessments.
- Develop and implement clear, goal-oriented care plans that address the client's health, care, functional, and quality of life needs.
- Serve as liaison with family and all involved professionals; navigate family dynamics to maximize success in achieving client goals.
- Provide ongoing assessment, oversight, and advocacy for clients to ensure needs are being addressed.
- Provide timely and thorough written reports and documentation of billable services.
- Collaborate with the Care Management team for professional development and support for department work processes.
- May transport clients as part of job responsibilities. Must maintain a valid driver’s license and meet the organization’s auto insurance requirements.
- Licensed MSW, MFT, MA Gerontology or related field highly desirable (minimum two years of experience).
- In-depth knowledge of the aging process, conditions affecting older adults, and care management best practices.
- Excellent communication skills (written and verbal), attention to detail, and time management abilities.
- Integrity, honesty, and commitment to high standards of excellence and professional ethics.
- Ability to work collaboratively with a multidisciplinary team.
- Availability all weekdays and some evening/weekend on-call rotation.
- Valid California driver's license, insurance, and personal transportation.
